Student Transfers

REQUIREMENTS FOR STUDENT TRANSFERS:

Parent Choice Student Transfer application FM-3281E Rev. (rev 04-18) signed by Principal/Administrative Designee of the home school.
Controled Open Enrollment Transfer application FM-7646 (rev 04-18) signed by Principal/Administrative Designee of the home school.

Parent/Guardian signature on the front and back of application

Working Parent Hardship: This applies to Elementary and Middle School students only

Original Employment letter
A signed statement from employer on letterhead with days and working hours of the parent(s). If there are several work locations please specify your location

Best Interest: This applies to all students

Please write a DETAILED explanation of the request for an alternate school.

Military:

Active duty military personnel/spouse must present a copy of their military or dependent identification.

ALL NECESSARY DOCUMENTS NEED TO BE SUBMITTED WITH THE APPLICATION OR IT WILL BE RETURNED.

*Please have the student identification number to get information on disposition of the transfer over the phone. All students, including Kindergarten, have a student ID number.

2018 - 2019 Parent Choice Student Transfer applications will be accepted beginning Monday, April 16, 2018.

2018 - 2019 Controlled Open Enrollment applications will be accepted in the window period of Monday, April 16, 2018 to Thrusday, May 31, 2018.

Controlled Open Enrollment applications will be available at your child's home school and faxed to the Region Office.

List of schools for Controlled Open Enrollment will be posted on www.dadeschools.net at that time.
